Case Manager
Focused on African American Community
AIDS Network seeks a full-time Case Manager to provide case management services for HIV-positive people in southern Wisconsin, specializing in the African American community. This professional position requires the ability to work effectively with diverse populations; comfort with issues of sexuality, substance abuse and mental health; strong knowledge of and familiarity with the African-American community and culture; the ability to provide culturally sensitive services; the capacity to maintain and establish relationships with internal and external service providers; ability to identify and assess needs and help clients and patients with structured service plans linking them to appropriate health care and services; capability to educate clients and community members; and ability to recruit for and facilitate support groups. Strong communication, time management, and computer skills required. Bachelors in Social Work or equivalent degree required. Experience with public benefits, health care, advocacy and problem solving desirable. Reliable automobile, insurance and valid driver's license required. Spanish speaking a plus. Good starting salary and benefits ($30-32k DOE).
